Leased network (umbrella network)

A PPO that rents its provider list to other insurers, so a dentist can be in-network with plans they never signed.

Also called: umbrella network

A leased or umbrella network is a PPO that rents access to its list of contracted dentists to other insurance carriers and third-party payers, who then market plans to their own customers claiming the dentist is in-network, even though the dentist only ever signed a contract with the original network, not with every company that later leases it. The practical result is that a practice can be contractually bound to a fee schedule for a plan whose name it does not recognize and never agreed to directly.

This structure is exactly why the write-off on a claim from an unfamiliar plan should be checked, not assumed. The original leased-network contract governs the terms, and if that contract has a fee schedule the practice never renegotiated, every carrier leasing that network inherits the same discounted rate, multiplying the effect of one old contract across a growing list of payers over time.

Reviewing which networks a practice's original PPO contracts allow to be leased, and under what terms, is a less obvious but genuinely valuable part of any fee schedule negotiation or PPO cleanup. It can affect far more of a practice's insurance mix than the original signed contracts suggest.
