Explanation of benefits (EOB)

The insurer's statement showing what was billed, what it paid, and what the patient or write-off covers the rest.

Also called: EOB

An explanation of benefits, or EOB, is the statement an insurance company sends after processing a claim, showing the fee billed, the plan's allowed amount, what the plan paid, the required write-off, and any remaining patient responsibility. It is the document that turns a submitted claim into an actual, line-by-line accounting of where the money went, and it is also where most underpayment hides in plain sight, because nobody is reading it closely enough to catch the error.

The habit that catches real money is comparing the allowed amount on the EOB against the plan's actual contracted fee schedule, procedure by procedure, rather than simply filing the EOB once payment posts. Insurers do apply the wrong fee schedule, downcode procedures, and misapply frequency limitations often enough that a practice auditing its EOBs regularly typically finds real, appealable underpayment that would otherwise have simply been absorbed as a write-off.

Assign EOB auditing to a specific team member with a specific cadence (weekly for a busy insurance mix) rather than leaving it as a task everyone assumes someone else is doing, since that is exactly the condition under which it stops happening.
